How to use this directory
The purpose of the Pediatric Care Coordination Directory is to provide health care practitioners and
agencies with a listing of agencies and individuals that provide care coordination services.
You can use the table of contents to quickly reference program overviews offered by each agency.
The index identifies which agencies provide particular care coordination services and are grouped
and listed by page number.
Individual database listings will provide you with a full description of agency contacts, phone
numbers, eligibility requirements, etc. – as well as whether or not the agency can link a family to
specific care coordination services.
It’s very important to note that a YES designated in the service box means that the agency links families
to the service. It does not necessarily mean that the agency actually provides that service. Blank boxes
within the agency’s database listing means that the agency does not link families to the identified
service.
